The Maryland Health Department requires that all enrolled children must meet minimum vaccination requirements for Diphtheria, Tetanus, and Pertussis (DTaP), Haemophilus Influenzae Type b (Hib), Hepatitis B, Measles, Mumps, & Rubella (MMR), Pneumococcal (PCV), Polio, and Varicella (Chickenpox), as outlined on this form. Maryland allows medical contraindication and religious objections to vaccination.
